Complex Injury & Functional Restoration

Plastic Surgery Trauma Associates functions within a Level I trauma center as the hospital-based reconstructive service for complex extremity salvage and industrial injury involving structural tissue loss.

This role is defined by trauma-system integration, staged operative sequencing, and coordinated multidisciplinary infrastructure rather than isolated procedural intervention.

Injury Profile

  • Structural instability following high-energy trauma
  • Combined soft tissue, tendon, and peripheral nerve loss
  • Perfusion compromise or uncertainty
  • Exposed bone or orthopedic hardware
  • Failure of prior reconstruction

Such injuries require hospital-based reconstruction with access to microsurgical capability, perfusion assessment, orthopedic coordination, and integrated rehabilitation services.

Staged Reconstruction

  • Microsurgical free tissue transfer
  • Peripheral nerve reconstruction
  • Tendon transfer or functional rebalancing
  • Free functional muscle transfer
  • Revision of failed operative sites
  • Deliberate staging across multiple procedures

The sequence and timing of these interventions directly influence complication risk, structural durability, and long-term functional capacity.

Isolated or improperly sequenced intervention may increase reoperation risk and prolong disability.

Functional Restoration

In industrial injury, durable structural repair and restoration of functional capacity are primary objectives.

  • Structured rehabilitation progression
  • Functional capacity evaluation
  • Coordinated maximum medical improvement determination
  • Longitudinal trauma-system follow-up

Trauma-Center Infrastructure

  • Dedicated microsurgical resources
  • Multidisciplinary operative coordination
  • Perfusion-guided assessment
  • Integrated inpatient and outpatient rehabilitation services

This systems-based framework supports complex limb salvage beyond routine procedural management.

Consultation

Consultation is appropriate for complex industrial and extremity injuries requiring staged reconstruction and trauma-system integration.

Early involvement influences structural durability and long-term functional capacity.
